Registration Form - This must be signed by all parties joining the school as part of the registration process.
Ballet Uniform Order Form - This contains the main items required for the ballet classes. Other items not found here can be ordered on the General Uniform Order Form. For sizing see the uniform page.
Tap Uniform Order Form - This contains the main items required for the tap and modern classes. Other items not found here can be ordered on the General Uniform Order Form.
Standing Order Form 2011/12 - If you wish to pay the full year fees for 2011/12 over 10 equal installments (2.5% discount applies). Please contact the principal for confirmation of totals.
